This article develops an economic and managerial approach to evaluating medical rehabilitation in Russian regions through the interaction of workforce capacity, financing, infrastructure development, and actual performance. The relevance follows from rapid expansion of patient coverage and equipment renewal alongside territorial differences in staffing, rehabilitation pathways, and asset utilization. The study aims to identify the factors that determine how budgetary and insurance resources are converted into accessible and sustainable rehabilitation services. The empirical base comprises official information from the Ministry of Health of the Russian Federation, the Federal Compulsory Medical Insurance Fund, the Accounts Chamber of the Russian Federation, federal budget documents, and regional health authorities for 2022–2025. Dynamic, comparative, index, and structural analysis were combined with regional panel comparisons before and after major equipment renewal cycles. The number of patients receiving medical rehabilitation increased from 1,140,831 in 2022 to 1,789,065 in 2025, representing growth of 56.8 percent. During the comparable period of 2022–2024, financial provision grew faster than patient coverage, while estimated financial intensity per patient rose by 34.2 percent. The share of outpatient rehabilitation remained nearly unchanged, indicating no pronounced nationwide transition toward a less resource-intensive model. Staffing levels below 70 percent were recorded in every third region, while 524 equipment units valued at 332.9 million rubles remained unused. Regional evidence revealed several management trajectories, including continued investment expansion, completion of procurement cycles, cluster-based routing, and concentration of advanced resources in supporting organizations. The proposed evaluation framework links three dimensions: financing, workforce readiness, and process-related performance. Its scientific contribution lies in separating formal infrastructure implementation from the real productivity of rehabilitation systems. Its practical value consists in providing indicators for regional planning, budget control, and accessibility management. The findings show that unsynchronized equipment procurement creates underused assets and weakens the conversion of public expenditure into measurable service capacity.
